Complete W-9 tax form (.pdf format - you can fill it out before printing) 5. Other documents required: A. Copy of Bond(s) - if required by your State or the States you want to work B. Copy of Business License or d/b/a - for all States that your are licensed or registered in C. Copy of State/County/City Collection/Occupational License - if required by your State/County/City laws D.
